Informed Consent – Savanna Psychology

COUNSELLING is a confidential process designed to help you address your concerns. There are defined rights and responsibilities held by each person.

 

CONFIDENTIALITY:

Counselling involves sharing sensitive, personal, and private information. All interactions with Rupinder Hehar, MC, R. Psych., including scheduling of or attendance of appointments, content of your sessions, progress in counselling, and your records are confidential.

 EXCEPTIONS TO CONFIDENTIALITY

  • If there is evidence of clear and imminent danger of harm to self and/or others, a psychologist is legally required to report this information to the authorities responsible for ensuring safety.
  • Any reporting of abuse or neglect of a minor (anyone under the age of 18), a psychologist is legally required to report this information to the authorities responsible for ensuring safety.
  • A court order, issued by a judge, may require a psychologist to release information contained in records and/or require the psychologist to testify in a court hearing.
  • If you give written permission for someone (e.g. doctor, school, lawyer) to have access to information, it will then be provided. This typically involves phone calls and written reports.

CONSENT TO RELEASE INFORMATION:

  • I consent to a report given to my insurance.
  • I consent to a report given to my family doctor.
  • I consent to information to be shared with my lawyer.

There is a charge of $225/hr unless other payment methods have been arranged. The session is scheduled for 1-hour, if you are late or end the session earlier, the session will be billed at the hourly rate, unless otherwise agreed. Clients seeking any type of assessment will be charged $300/hr unless other payment methods have been arranged. Clients that require additional reports will be charged at a rate of hour, unless other payment methods have been arranged. A 24-hour notice is appreciated for cancellations. If there is no cancellation or cancellation is less than 24 hours, a fee of $150 will be charged. Clients will be charged an appropriate fee for any preparation time that is required to comply with formal or informal requests, case conferences, and extended phone calls or email responses. Clients take sole responsibility to seek reimbursement from their insurer unless a third party has taken responsibility for payment. If there is a third party payer (insurance company), they may be given information about the type(s), cost(s), date(s), and services provided.

If you have an emergency, you should call the Distress Centre at 403-266-1601 or take yourself to the nearest hospital emergency.

I have read and discussed the above information with Rupinder Hehar. I understand the risks and benefits of counselling, the nature and limits of confidentiality, and what is expected of me as a client.
